OMPRIS - Online-based motivation program for the reduction of problematic media consumption and enhancement of treatment motivation for individuals with computer game addiction and internet addiction

The WHO's recognition of computer gaming addiction (Gaming Disorder) and the needs of patients require developing low-threshold, innovative and disruption-oriented help services. The OMPRIS multi-center care research project, which is funded by the Innovation fund of the federal joint committee (G-BA), aims to counteract the development of addiction or chronification of Internet-related disorders through tele-medical interventions at an early stage. The project uses a specially developed webcam-based motivation program. This is intended to create motivations for behavioral change and reduce addiction symptoms. The program can be used free of charge. Participants can use the program at the place of their problem behavior, the Internet itself. In addition to the disorder-oriented therapeutic intervention, there is also the possibility of webcam-based social counseling.

Inclusion criteria:

  • Possibility of regular use of an internet connection with webcam as well as an email address
  • Age >= 16 years (under 18 years with the consent of at least one parent in form of a short written consent)
  • Sufficient understanding and speaking of the German language
  • Consent to de-pseudonymization in case of emergency (e.g. acute suicidal tendencies during counseling sessions)

Exclusion criteria:

  • Acute psychotic symptoms (acute delirium, hallucinations, massive thinking disorders)
  • Acute suicidality or self-endangerment
  • Severe intelligence impairment that precludes participation
  • Substance dependence in the foreground
  • Age under 16 years or lack of consent of the legal guardian for minors
  • Lack of German language skills
  • Lack of regular Internet access with webcam and lack of email address
  • Presence of known somatic pre-existing conditions, which – or their treatment with hormone-influencing drugs – could be associated with impulse control disorders (e. g. Parkinson's disease with dopaminergic medication)
  • Current psychiatric or psychotherapeutic treatment due to media addiction

SCAVIS - Stepped Care Approach for the treatment of Internet-related disorders

The SCAVIS project is designed as a holistic, stepped care approach and is supported by the innovation fund (Innovationsfonds). This Employee Assistance Program offers adequate assistance for the spectrum of Internet-related disorders (IRD). The stepped care approach consists of a screening and tracking app (stage 1), brief telephone consultation (stage 2) and online therapy (stage 3). All interventions are e-health. As part of the SCAVIS project, an independent module Online Therapy (Step 3) for the treatment of IRD will be implemented in Mainz and Berlin in cooperation with Prof. H. Salbach (Freie Universität Berlin) starting autumn 2020. The online therapy is based on a standardized behavioral therapy treatment manual, which is based on cognitive behavioral therapy.

Inclusion criteria:

  • Women and men from companies between 16 and 64 years of age with internet-related disorders - comorbid psychiatric diagnoses possible, recorded via tracking app
  • (Step 1): Compulsive Internet Use Scale (CIUS) >20 points
  • (Step 2): Brief intervention by telephone. Missing success: Step 1 and 3, criteria according to DSM-5 or 1 criterion according to ICD-11
  • Online therapy. Lack of success: Step 2 and >4 criteria according to DSM-5 or 3 criteria according to ICD-11 as well as already in the first survey > 6 criteria DSM-5

Exclusion criteria:

  • Insufficient knowledge of the German language
  • If taking psychotropic medication, no change in dosage in the two months before and during the SCAVIS online therapy intervention
  • If taking psychotropic medication, change in dosage in the two months before and during the SCAVIS Online Therapy intervention; taking psychotropic medication <4 weeks before
  • Further psychotherapeutic treatment (completion at least 4 weeks before the start of SCAVIS)
